The Challenge
Segimed was born from the need to move away from paper-based processes and fragmented communication between patients and doctors specializing in pulmonary hypertension. Without a centralized system, clinical histories were scattered, emergency consultations depended on calls and emails, and continuous monitoring of vital signs and agile coordination of the medical team were nearly impossible.
The Solution
We developed a mobile and web platform that completely digitizes the care cycle for patients with pulmonary hypertension, integrating:
- Digital clinical history: Each patient creates and updates their profile with studies and diagnoses, sharing it securely with their medical team.
- 24/7 monitoring and connected alarms: Integration with devices (blood pressure monitors, oximeters, scales) via Bluetooth to send real-time alerts for critical values.
- Immediate intercommunication: Direct chats and calls between patient and doctors, speeding up consultations, teleconsultations, and medical orders.
- Smart appointment scheduling: Quick booking of appointments, in-person or virtual, showing real-time availability.
- Continuous advice and education: Interactive content on therapies and self-care, plus support from a team of advisors to optimize app usage.
- Security and traceability: Encrypted data, access tokens, and double verification ensure privacy and the recording of every interaction.

Our Strategy
Clinical workflow mapping: we identified every point of contact between patient and specialists to eliminate friction. Scalable architecture: we designed independent modules (history, monitoring, communications, scheduling), ready to incorporate new specialties or devices. Medical hardware integration: we connected wireless sensors to automatically capture vital data. Automation of alerts and reports: pipelines that send instant notifications and generate records of critical events for auditing. Training and support: we trained doctors and patients in using the platform, ensuring high adoption and trust.

Results
Segimed completely digitized the monitoring of pulmonary hypertension: patients carry their clinical history at all times and report their vital signs in real time; the medical team coordinates treatments and responds to emergencies in minutes, not hours; paper and spreadsheets were eliminated, unifying all information in a single secure system; and the continuous data capture lays the groundwork for clinical studies, trend analysis, and constant improvement of care protocols.
